Arizona National Guard – Archives and Official Records
The Records Management and Records Holding Area (RM-RHA) office manages and maintains the Archives and Official records of the Arizona National Guard.
The Records Management office provides assistance to Service Members (SM) and their designated representatives that request copies of their Official Military Personnel File (OMPF).
The RM-RHA office maintains the records those who served in the Arizona Army and Air National Guard from 2004 and earlier. The RM-RHA office can also assist with request for personnel that served and departed from the Army NG after 2005. For personnel that served and departed from the Air National Guard after 2004, request for service records can be submitted to the Air Reserve Personnel Center (ARPC) for assistance.
Requesting service records
- Service Members, retrieve, fill out, sign and submit a Standard Form (SF) 180
- When submitting a request on behalf of a Service Member, include copies of the Power of Attorney, authorization for release or death certificate
- Submit request to the AZ Records Management and Records Holding Area office via e-mail, USPS or in Person.
